Patient Forms
We encourage you to print the Patient Forms and complete them at home, prior to your appointment with Dr Barth. Please bring the forms with you on your appointment day.
Please click on the Form you would like to access. When the file opens, select Print from the File menu in your browser to print the form. Please print and sign the Patient Form, Financial Agreement, and HIPPA Policy.
